Note: To ensure your appliances are delivered on time, please be sure to fill in the desired Due Date and exact appointment time on the prescription form. This enables us to properly schedule your case and ensure you receive delivery on time.
Mailing Labels
OC offers three convenient options for submitting and delivering orders:
1. You can mail us your orders via the U.S. Mail using pre-paid mailing labels that we provide.

2. We can arrange a regular schedule to pick up and hand-deliver your orders. This option eliminates the possibility of an uncertain delivery schedule through the U.S. Mail. To arrange a schedule, please contact our office.
3. Submit your digital scans to OrthoCreations. See the Digital Services subject for more information about forwarding your stl files.
Note: Adobe Acrobat Reader or similar program must be installed on your computer in order to open and print the Prescription Forms and Mailing Labels.
