1. How to mail a case
- Complete the provided RX form, or print from the website, and include the Due Date and exact appointment time for RUSH orders.
- Wrap each impression or model separately and attach the RX with a rubber band.
- Pack the item securely with bubble wrap or other cushioning material.
- Securely tape the outside of the box. Add the OrthoCreations pre-paid mailing label and the return address if desired.
2. Mailing models or alginate/compound impressions for appliances
- Please wrap WET alginate impressions in an airtight zip-lock baggie.
- Please do not place the paper RX inside the airtight bags. Wrap the RX around the exterior of the bag with a rubber band or staple to the bag.
- Opposing model is required when any desired corrections may cause a traumatic occlusion.
- Wax bite is recommended when supplying the opposing model.
3. Delivery Time expectations
- Normal delivery time is 2-5 “in-lab” business days.
- Appliances can be delivered in 48 hours if noted on the RX Due Date.
- RUSH orders can be delivered same or next day for a minimal fee. Doctors have the choice to pay for overnight shipping.
- Please call in advance to make arrangements for RUSH orders.
4. Do you seat bands?
- OC has bands on-hand, with or without tubes. For digital models, the doctor can choose to supply the bands to reduce the cost of the appliance. When sending a model, the office can choose to insert the bands themselves or OC can provide and set the bands.
5. Do you pour impressions?
- Yes, for a nominal $5.00 charge.
6. Shipping
- OrthoCreations provides pre-paid United States Postal Service mailing labels and shipping boxes.
- Customers are responsible for paying for return shipping to the doctor. OrthoCreations charges First Class or Priority USPS mail rates for return shipping.
- Appliances can be shipped for next day delivery, upon special request. An additional charge will apply and be paid by the doctor’s office. OrthoCreations does not pay for FedEx.
7. Billing & Payments
- Payment is due within 10 business days of the invoice date.
- Invoices are included with every delivery to include the patient name, the appliance, the due date and the price per appliance.
- If payment cannot be remit within 10 business days of delivery, a monthly statement will be mailed.
- The monthly payments are due within 30 days following the month in which they are billed. If payment is not made, a 5% late fee will be assessed
8. Repairs & Remakes
- Repairs will be done at little or no charge, depending on the nature of the damage.
- Please return the original appliance when requesting a remake. Write a detailed note as to why a remake or repair is needed. OC will review the lab quality of the item and if patient negligence is the reason for the remake; for example, the dog chewed the retainer or the patient cracked the acrylic with due force. OC will determine if the remake is done at NO CHARGE within 90 days or if a prorated charge applies.
